Medical Marijuana certifications

Medical Cannabis became legal in Florida in 2016, after Amendment 2 was passed (Florida Medical Marijuana Legalization Initiative). Florida residents must first qualify for medical marijuana certification before purchasing a product at a marijuana treatment center. 

At Your Family MD, we guide you through the process of certification,  we personalize and optimize your treatment plan. We are here to help patients with chronic or debilitating medical conditions, who haven’t been able to find adequate treatment with traditional medicine, obtain legal access to medical marijuana in Florida. 

Patients may qualify for medical marijuana in the state of Florida if they have been diagnosed with the following conditions:

-    Cancer

-    Epilepsy

-    Glaucoma

-    HIV / AIDS

-    PTSD

-    Parkinson’s Disease

-    Terminal condition

-    ALS

-    Multiple Sclerosis

-    Crohn’s disease

-    Chronic nonmalignant pain

During the video call, we will discuss the required documentation that you will need to have available before your appointment for certification and the steps that you need to follow. 

First steps that you will need to do after selecting a day for a video call include:

-    Complete the New Patient Intake Form and return it to us before your appointment.

-    Please provide us with your medical records (current within the last 12 months). There are various ways of obtaining records.  You can have your primary care physician or specialist, fax a copy of your records to us at, 844-863-8737. You can utilize our Record’s Release Form to provide it to your doctors or provide it to us and we will fax It to your doctor. 

Once we receive your records and the signed forms, we will contact you to schedule an appointment for physical examination and certification. Important: all pages must be completed and returned to our office before scheduling your appointment.
